To apply for Support at Home, contact My Aged Care on 1800 200 422 or visit their website to create a profile and arrange an assessment. Following your assessment, you will be informed of your eligibility and the level of support approved. Once approved, you can choose My Guardian as your provider, and we will work with you to develop a personalised care plan tailored to your needs.

Support at Home provides a wide range of services designed to help older Australians live independently at home. These may include personal care, assistance with daily living activities, nursing and allied health services, medication management, meal preparation, housekeeping, transport, and social support. Services are tailored to meet your individual goals and care needs.

Your level of support is determined through an assessment conducted by My Aged Care. Based on your individual needs and circumstances, you will be approved for a level of funding and services appropriate to your situation. This ensures you receive the right level of support to help you remain living independently at home.
