To begin your journey with aged care, the first step is to apply through My Aged Care, the Australian Government’s portal for aged care services. If you are aged 65 or above, you can apply for a Home Care Package. You can start your application online or by calling My Aged Care at 1800 200 422.
Once your application is submitted, it’s important to request an assessment. A representative from My Aged Care will evaluate your physical, emotional, and social needs. This assessment is an opportunity to discuss your preferences and any specific support you may require for a tailored care plan that meets your needs.
After the assessment, you will receive a summary of the findings and recommendations, including whether you’ve been approved for a package. We will work closely with you to develop a personalised care plan that outlines the specific services you’ll receive, including daily assistance, medical support, social activities, and companionship.

Packages are allocated fairly through the national priority system, based on date of approval and priority for
care. You are placed in the system from the date stated on your approval letter.
Home Care Packages include a range of services such as personal care, assistance with daily living activities, medication management, health services, meal preparation, housekeeping, transportation, and social engagement activities. The services are tailored to meet your specific needs.
The level of support is determined through a physical assessment from My Aged Care. Based on this assessment, you will be assigned one of four levels of care, ranging from basic support to high-level care, ensuring you receive the appropriate level of assistance for your needs.
